The lungs, long understood to be primarily vulnerable to the effects of smoking, are now recognized as susceptible to damage from more widespread health concerns: obesity and chronic inflammation. A recent Brazilian study, following nearly 900 individuals, reinforces the idea that lung health isn’t solely determined by whether or not someone smokes, but also by metabolic and inflammatory processes occurring throughout the body. This research sheds light on the complex factors contributing to chronic obstructive pulmonary disease (COPD), a condition increasingly understood as far more than simply a “smoker’s disease.”
COPD, characterized by inflammation and thickening of the airways, leads to shortness of breath and progressive respiratory limitations. While cigarette smoking remains the leading cause, the study published in BMC Pulmonary Medicine demonstrates that obesity and systemic inflammation can independently contribute to lung deterioration. This is particularly significant as it suggests that younger individuals, even those who have never smoked, may be at risk for developing early signs of lung dysfunction.
Researchers tracked participants from the “Coorte de Nascimentos de Ribeirão Preto,” a group born between 1978 and 1979, measuring their lung function twice – once between ages 23 and 25, and again between 37 and 38. The findings revealed a clear correlation between several factors and declining lung health. Smoking had the most significant impact, associated with an average 1.95% reduction in lung function over the 12-year period. Still, systemic inflammation, measured by levels of C-reactive protein (CRP) in the blood, corresponded to a 0.76% decline in lung function for each 1 mg/dL increase in the inflammatory marker. Obesity also played a role, with each 1 kg/m² increase in body mass index (BMI) resulting in a further 0.28% reduction in lung function. The study was supported by the São Paulo Research Foundation (FAPESP).
The Link Between Systemic Inflammation and Lung Damage
According to Elcio Oliveira Vianna, a professor at the University of São Paulo’s Ribeirão Preto Medical School and the study’s coordinator, the research builds upon previous findings suggesting that processes beyond smoking significantly impact lung health. “The findings reinforce what previous studies with smaller cohorts had already shown: in addition to the effects of cigarettes, metabolic and systemic inflammatory processes can play an important role in the deterioration of lung function, even in younger individuals without diagnosed respiratory diseases,” Vianna stated, as reported by Agência FAPESP.
Vianna explained that systemic inflammation originating from other parts of the body – such as inflammation within fat cells in cases of obesity – can directly interfere with lung function. “Systemic inflammation of low grade, already known to increase the risk of cardiovascular diseases, also affects and damages the lungs. This constant, even subtle, inflammatory bombardment, as in the case of obesity, contributes to tissue damage over time, potentially triggering premature lung aging,” he said.
Rethinking COPD: A Multifactorial Disease
The study’s findings challenge the traditional view of COPD as solely a disease of smokers. Researchers observed early signs of the disease in participants under 40, an age group typically outside the diagnostic window for COPD. This suggests that obesity and systemic inflammation can increase the future risk of developing the condition. “The systemic inflammation has a direct impact on lung function and we were able to demonstrate this in this population study. Because all participants were young, it was possible to identify signs of the disease even before its clinical diagnosis,” Vianna noted.
Interestingly, the connection between COPD and obesity isn’t always readily apparent. “One of the consequences of COPD is loss of appetite and high caloric expenditure [loss of fat and lean mass] due to the effort required to breathe. As COPD patients are generally thin, it is not common to associate the disease with obesity. That’s why it was important for our study to demonstrate that obesity, like other systemic inflammations, can trigger the disease,” Vianna explained.
Ana Carolina Cunha, a pulmonologist and the study’s first author, emphasized the broader implications for understanding COPD. “The disease is multifactorial and much more complex than previously thought. In addition to the inflammation caused by cigarettes, there may be a systemic inflammatory process specific to the individual. Previous studies had already pointed to this association. Today we know that COPD patients have chronic inflammation, which raises the hypothesis that this process may be a common factor in different manifestations of the disease, especially in people with genetic or metabolic predisposition.”
The research highlights the importance of addressing obesity and systemic inflammation as potential preventative measures for lung disease, even in individuals without a history of smoking. While further research is needed to fully understand the mechanisms at play, this study provides compelling evidence that lung health is influenced by a wider range of factors than previously recognized.
