Pregnancy as “Early Warning System” for Heart Health | University of Freiburg Study

Freiburg, Germany – A routine pregnancy may hold a surprising key to a woman’s long-term cardiovascular health, according to latest research from the University of Freiburg and Danish colleagues. A study published February 18, 2026, in JAMA Cardiology reveals that certain blood markers measured during pregnancy can predict a woman’s risk of developing heart disease – even decades later. This discovery is prompting a new approach to preventative care, with the University of Freiburg now offering a specialized follow-up clinic for women who experienced high blood pressure during pregnancy.

The research team analyzed blood samples from over 2,000 pregnant women, linking those results to their health data over a 12-year period. They found that women with elevated levels of a specific biomarker, sFlt-1, during the third trimester, along with those who experienced pregnancy-induced hypertension, were at a significantly higher risk of developing cardiovascular diseases like stroke or heart failure. Remarkably, this increased risk was observed even in women who were previously considered healthy.

Pregnancy as a “Stress Test” for the Heart

“Pregnancy acts like a natural stress test for the heart,” explains Professor Dr. Dirk Westermann, Medical Director of the Clinic for Cardiology and Angiology at the University of Freiburg. “With our findings, we can improve women’s health in the long term, because cardiovascular disease remains the leading cause of death for women.” Dr. Lucas Bacmeister, a cardiologist at the same clinic and the study’s lead author, adds, “Our study shows that indicators of increased cardiovascular risk can be identified during pregnancy – long before symptoms appear.”

The study built upon data from a large cohort study in Southern Denmark, encompassing 38,455 pregnant women. Within that group, researchers focused on 2,056 women with available blood marker data. Over the 12-year follow-up period, 28 women developed cardiovascular disease, with an average age of just 40 at the time of diagnosis.

sFlt-1 and Preeclampsia: Key Indicators

Elevated levels of sFlt-1, a protein involved in blood vessel development, in the final trimester of pregnancy, coupled with pregnancy-induced hypertension, were the strongest predictors of future heart problems. The combination of age and this biomarker proved more accurate in assessing risk than traditional factors like blood pressure or cholesterol levels alone. Approximately six to ten percent of all pregnancies are affected by pregnancy-induced hypertension, with preeclampsia being the most common form.

New Clinic Addresses a Critical Gap in Care

Recognizing the importance of these findings, the University of Freiburg has established a specialized follow-up clinic to provide comprehensive care for women who have experienced pregnancy-related hypertension. The clinic is open to all women with a history of high blood pressure during pregnancy, even if they did not deliver at the University of Freiburg hospital.

“We are creating a clear point of contact for women after pregnancy-induced hypertension,” says Dr. Bacmeister, who coordinates the clinic. “With standardized appointments a few weeks after delivery and again after one year, we can identify those who need closer monitoring and intervene accordingly.” The clinic operates in close collaboration with the university’s Department of Gynecology, ensuring a holistic approach to patient care.

Professor Dr. Ingolf Juhasz-Böss, Head of the Clinic for Gynecology at the University of Freiburg, emphasizes the importance of structured follow-up care. “This is an important step in keeping the long-term health of our patients in focus,” he says. A scientific registry will accompany the clinic, allowing researchers to continuously improve and refine the program.

The findings underscore the need for increased awareness of cardiovascular risk factors during and after pregnancy. While more research is needed to fully understand the mechanisms at play, this study offers a promising new avenue for preventative cardiology, potentially saving lives by identifying and addressing risk factors early on. Women concerned about their cardiovascular health after pregnancy are encouraged to discuss their individual risk factors with their healthcare provider.
