While continuous positive airway pressure (CPAP) therapy remains the gold standard for treating obstructive sleep apnoea, adherence rates are surprisingly low. Nearly two-thirds of patients discontinue CPAP therapy, highlighting the challenges of long-term compliance. Alternative treatments, such as weight loss medications, oral appliances to maintain an open airway, and hypoglossal nerve stimulation implants, are available, but aren’t suitable for all patients. Weight loss drugs are showing promise, but are not a universal solution.

The study’s call for workplace screening raises complex considerations. While early detection could lead to significant economic benefits, concerns exist about potential discrimination. Adrian Jones, national officer at the trade union Unite, which represents many transport workers, emphasized the need for safeguards. “Professional drivers often hide issues of sleep apnoea out of fear of losing their jobs,” he said. “Before introducing screening, employers must be fully educated that sleep apnoea can be effectively managed. Workers must have the confidence that they will be supported, not dismissed, if a problem is identified.”

The authors of the Thorax study advocate for policymakers to prioritize the development of effective screening strategies and public health campaigns to raise awareness about obstructive sleep apnoea. Early identification, coupled with proactive follow-up care, could unlock substantial savings and improve the health and well-being of millions. The economic implications are clear: addressing this widespread, yet often overlooked, condition is not just a matter of public health, but as well a sound economic investment.

The potential for new, low-cost technologies and screening algorithms offers a glimmer of hope for improving diagnosis rates.
