President Donald Trump said he plans to send $90 checks to more than 20 million seniors this month. The one-time payments are headed to most of the nation’s seniors enrolled in Medicare Part B, spanning all 50 states according to a White House breakdown reviewed by journalists.

Most eligible recipients will receive the money by direct deposit in early October, while others will get a mailed check. The standard monthly premium for Medicare Part B is $202.90 in 2026, meaning the $90 rebate covers close to half of a single month’s payment.

Funding Sources and Eligibility Rules

The White House confirmed that the payments will come from the Medicare Improvement Fund, which was established by Congress in 2008 to finance enhancements to the traditional Medicare fee-for-service program. Larry Levitt, the executive vice president for health policy at the healthcare research nonprofit KFF, noted that previous administrations had never used the fund in this specific manner, although lawmakers have periodically withdrawn money from it for other budgetary priorities, explaining that the statutory language governing the fund is quite broad and grants substantial discretion to the administration. The Trump administration stated that the fund holds approximately $2 billion and that this marks the first time it has been used to provide direct cash payments to beneficiaries according to the administration’s figures.

In his social media announcements, Trump criticized how prior administrations managed the reserve. This money will come from the Medicare Improvement Fund, a pointless Slush Fund that has only been used by Dumocrats in Congress to pay for the Waste, Fraud, and Abuse for their Special Interest friends, and drive up Healthcare costs he wrote on Truth Social.

Not all Medicare enrollees qualify for the payout. More than half of eligible Medicare beneficiaries are enrolled in Medicare Advantage, as reported by KFF. Individuals whose health care premiums are already paid by Medicaid, those who pay an income-related monthly adjustment amount, and seniors enrolled in private Medicare Advantage plans are excluded from the distribution according to the Centers for Medicare and Medicaid Services.

State-by-State Rebate Allocations

The distribution spans all 50 states, with larger states receiving the highest total counts of checks based on the White House breakdown. Wyoming, Vermont, Alaska, North Dakota, and South Dakota are each slated to receive fewer than 100,000 checks apiece for their older residents.

In several closely contested Senate battleground states, significant numbers of seniors are also in line to receive the funds as reported by USA Today, including 748,000 in Ohio, 644,000 in North Carolina, 568,000 in Georgia, 539,000 in Michigan, 327,000 in Iowa, 293,000 in Kansas, and 88,000 in Maine.

Administration Issues Payouts Before November Midterm Elections

The announcement arrives exactly one month before the November 3 midterm elections, as the administration deploys direct financial relief amid voter anxieties regarding inflation and the cost of living. Critics have characterized the timing as a political maneuver, with Sen. Ron Wyden calling the payments Trump’s latest attempt to bribe the American public.

The senior payout follows other recent financial distributions from the administration, including $500 refund checks sent to nearly 1 million Americans across 30 states for what officials described as overcharged fees through the Affordable Care Act federal exchange. Trump has campaigned on a proposal to distribute $5,000 dividend payments to every U.S. citizen if Republicans retain control of the House and Senate in the midterms.
