Personalized Vitamin D3 Therapy Slashes Risk of Repeat Heart Attack, Landmark study Finds

A personalized vitamin D3 treatment plan, tailored to individual blood levels, can significantly reduce the risk of a second heart attack in patients who have already suffered one, according to groundbreaking research presented on November 9 at the 2025 American Heart Association Scientific Sessions in New Orleans. The large-scale clinical trial revealed a 50% reduction in the likelihood of another cardiac event through a “target-to-treat” approach.
The Promise of Precision Vitamin Therapy
For decades, a link has been observed between low vitamin D levels and increased risk of cardiovascular disease. However, previous attempts to mitigate this risk with standard vitamin D supplementation yielded disappointing results. Researchers at Intermountain Health in Salt Lake City hypothesized that a more precise, individualized approach – one that actively monitors and adjusts dosage to achieve optimal blood levels – could unlock the potential of this vital nutrient.
“Previous studies just gave patients supplementation without regularly checking blood levels of vitamin D to determine what supplementation achieved,” explained a senior researcher involved in the study. “with more targeted treatment, when we checked exactly how supplementation was working and made adjustments, we found that patients had their risk of another heart attack cut in half.”
Inside the TARGET-D Trial
The TARGET-D trial, conducted between April 2017 and May 2023, enrolled 630 patients who had experienced a heart attack within the prior month.Participants were followed until March 2025, with one group receiving standard care and the other undergoing active, targeted vitamin D3 treatment.
The treatment group aimed to elevate blood vitamin D levels above 40 nanograms per milliliter (ng/mL). At the study’s outset, a striking 85% of participants had levels below this threshold. Achieving this target required a significantly higher initial dose for many patients – over half received 5,000 international units (IU) of vitamin D3 daily, compared to the typical recommended intake of 600-800 IU.
Dosing, Monitoring, and Key Findings
Maintaining optimal vitamin D levels involved regular blood tests. Patients with healthy levels were monitored annually, while those with deficiencies were tested every three months, with dosage adjustments made until the 40 ng/mL target was reached. Once achieved, levels were monitored annually.
researchers tracked major cardiac events (MACE) – including heart attacks, strokes, heart failure hospitalizations, and deaths.While the study did not reveal a significant difference in the overall risk of MACE between the two groups,the results demonstrated a remarkable 50% reduction in the incidence of second heart attacks among those receiving targeted vitamin D treatment.
“We observed no adverse outcomes when giving patients higher doses of vitamin D3 supplementation, and to significantly reduce the risk of another heart attack, which are exciting results,” said Heidi May, PhD, cardiovascular epidemiologist at Intermountain Health and the study’s principal investigator. “We’re excited with these results but know we have further work to do to validate these findings.”
Global Implications and Future Research
The findings carry significant global weight, given that an estimated one-half to two-thirds of the world’s population suffers from vitamin D deficiency. Historically,sunlight exposure provided sufficient vitamin D for most individuals. Though, increased awareness of skin cancer risks and lifestyle changes have led to reduced sun exposure, necessitating reliance on dietary supplements.
Researchers are now planning a larger clinical trial to confirm and expand upon these promising results. A larger study group will allow for a more thorough evaluation of whether targeted vitamin D management can reduce not only repeat heart attacks but also other forms of cardiovascular disease. this research represents a pivotal shift towards precision medicine in the fight against heart disease, offering a potentially simple and effective strategy for improving patient outcomes worldwide.
