The World Health Organization (WHO) is urging a global acceleration in the fight against tuberculosis (TB), a disease that continues to claim over 3,300 lives daily. Central to this renewed push are newly endorsed diagnostic tools designed to bring faster, more accessible testing to communities worldwide. These innovations, including point-of-care tests and a novel tongue swab method, represent a significant step toward ending TB, a preventable and curable disease that still affects millions. The focus on rapid diagnosis is critical, as delays in treatment contribute to increased transmission and poorer patient outcomes.
Tuberculosis remains a formidable public health challenge, particularly in low- and middle-income countries. According to the WHO, more than 29,000 people fall ill with TB each day. While global efforts have saved an estimated 83 million lives since 2000, progress is now threatened by funding cuts and logistical hurdles in delivering care. The new WHO guidelines aim to address these challenges by simplifying testing procedures and reducing costs, making diagnosis available closer to where people seek care. This is particularly important for individuals who may not have simple access to centralized laboratories.
Bringing Diagnosis Closer to Patients
The newly recommended point-of-care diagnostic tests are portable, battery-operated, and can deliver results in under an hour – a substantial improvement over traditional methods that often require days or weeks for results. These tests are similarly significantly more affordable, costing less than half of many existing molecular diagnostics. “These new tools could be truly transformative for tuberculosis, by bringing fast, accurate diagnosis closer to people, saving lives, curbing transmission and reducing costs,” said Dr. Tedros Adhanom Ghebreyesus, WHO Director-General. The WHO’s statement emphasizes the urgent require for countries to scale up access to these and other innovative tools.
Beyond speed and cost, the new guidelines also introduce a significant advancement in sample collection. For the first time, adults and adolescents who struggle to produce sputum – a common requirement for TB testing – can be tested using a simple tongue swab. This is particularly crucial as individuals unable to produce sputum are at a higher risk of dying from the disease. The guidelines also recommend sputum pooling, a strategy where samples from multiple individuals are combined and tested together, reducing costs and accelerating results, especially in resource-constrained settings.
Expanding Diagnostic Capabilities Beyond TB
The potential of these new diagnostic platforms extends beyond tuberculosis. The devices are designed to be adaptable and can be used to test for other infectious diseases, including HIV, mpox, and human papillomavirus (HPV). This versatility positions them as valuable assets in strengthening broader public health infrastructure and creating more integrated, patient-centered care systems. The ability to offer multiple diagnostic tests at the point of care streamlines healthcare delivery and improves efficiency, particularly in areas with limited resources.
The Challenge of Funding and Implementation
Despite the promise of these new tools, significant challenges remain. Global funding for TB research and implementation lags far behind the estimated need of around US$5 billion annually. According to the WHO, cuts in global health funding pose a serious threat to reversing the gains made in TB control. Uptake of rapid diagnostic tools has also been hampered by high costs and reliance on centralized laboratories, highlighting the need for sustained investment and strategic implementation.
Dr. Tereza Kasaeva, Director of WHO’s Department for HIV, Tuberculosis, Hepatitis and Sexually Transmitted Infections, underscored the economic benefits of investing in TB control, stating, “Investing in TB is a strategic political and economic choice, generating up to US$ 43 in health and economic returns for every dollar spent.” This return on investment highlights the importance of prioritizing TB as a central pillar of health security and universal health coverage.
Looking Ahead: World TB Day 2026 and Beyond
As countries prepare to mark World TB Day in 2026, under the theme “Yes! We can end TB: Led by countries, powered by people,” the WHO is calling for urgent action on multiple fronts. These include accelerating the rollout of new diagnostic technologies, strengthening people-centered TB care with community leadership, building resilient health systems, tackling the social and economic drivers of TB, and protecting essential services amid global crises. The WHO is also actively supporting research and innovation through initiatives like the TB Vaccine Accelerator Council, which aims to fast-track the development and equitable access to new TB vaccines.
The path to ending TB requires a sustained, coordinated global effort. While these new diagnostic tools represent a critical step forward, continued investment in research, implementation, and equitable access will be essential to achieving the ambitious goal of a TB-free world.
