Understanding Pneumococcal Disease and Conjugate Vaccine Protection
Pneumococcal disease is a serious infection caused by Streptococcus pneumoniae bacteria, commonly referred to as pneumococcus, according to the Centers for Disease Control and Prevention. The bacteria can trigger a wide range of illnesses, including ear and sinus infections, pneumonia, and bloodstream infections. According to the Children’s Hospital of Philadelphia, severe complications of the infection can include empyema, which involves pus building up between the lung and the chest wall; sepsis, a dangerous bloodstream infection; and meningitis, an inflammation of the lining surrounding the brain and the spinal cord.
To protect against these outcomes, modern conjugate vaccines link pneumococcal polysaccharides to a harmless helper protein. This design encourages children to mount an immune response to the helper protein while simultaneously generating antibodies against the bacterial polysaccharides. The result is that children are protected against pneumococcus without risking natural infection, which can lead to permanent disabilities or death. Beyond protecting the individual, conjugate vaccines also reduce the quantity of bacteria found in a vaccinated person’s nose, effectively decreasing the community spread of the pathogen, according to the Children’s Hospital of Philadelphia.
Available Conjugate and Polysaccharide Vaccines for Children and Adults
The U.S. Food and Drug Administration has approved multiple pneumococcal vaccines over the decades. The first polysaccharide vaccine was licensed in 1977 and protects against 23 types of pneumococcal bacteria. Pneumovax 23 (PPSV23) was approved in 1983 and protects against 23 types of pneumococcal bacteria. For conjugate vaccines, older versions containing fewer types, such as PCV7 and PCV13, are no longer available, according to the Children’s Hospital of Philadelphia. Newer formulations have expanded coverage significantly.
PCV15 protects against 15 types of pneumococcal bacteria and was approved by the FDA in 2021 for adults and 2022 for children.
PCV20 protects against 20 types of pneumococcal bacteria and was approved in 2023 for use in people 6 weeks and older.
PCV21 was approved in 2024 for adults aged 19 and older, designed specifically to protect against eight new types of pneumococcal bacteria commonly causing severe illness in adults that are not included in other licensed vaccines.
Safety Monitoring, Common Side Effects, and Rare Risks
Federal agencies including the CDC and the FDA continuously monitor pneumococcal vaccines using safety systems.
The most common side effects reported after pneumococcal vaccination are mild, lasting for one or two days. According to the CDC, these include feeling drowsy, loss of appetite, soreness or swelling at the injection site, fever, and headache. Similarly, the Children’s Hospital of Philadelphia notes that potential vaccine risks include pain, redness, and swelling at the injection site, a high fever exceeding 102 degrees Fahrenheit in up to 10 out of 100 recipients, decreased appetite, and irritability. Because serious side effects do not occur often, health authorities emphasize that the benefits of the vaccine clearly outweigh its risks.
However, specific combinations of vaccines can introduce distinct temporary risks. Young children who receive both the pneumococcal vaccine and the inactivated flu vaccine (shot) at the same time may be more likely to experience a febrile seizure, which is a seizure associated with a high fever. Although described as scary, these febrile seizures do not cause long-lasting effects, according to the Children’s Hospital of Philadelphia. Additionally, severe allergic reactions following vaccination are rare, but anyone showing signs of an anaphylactic reaction—such as hives, swelling, or difficulty breathing—is advised to call 911 immediately.
Vaccination Guidelines for Children and High-Risk Adults

Beyond infancy, vaccination schedules adapt depending on age, health status, and prior vaccination history. Adults aged 50 years and older who have not been previously vaccinated against pneumococcus are advised to receive either PCV20 alone, PCV21 alone, or a combination of PCV15 followed by a dose of PPSV23. For those previously vaccinated, individuals should consult a healthcare provider to determine whether another dose or a specific version is required.
Younger adults between the ages of 19 and 49 also face elevated risks if they live with certain underlying conditions. Individuals with immunocompromising conditions, cochlear implants, cerebrospinal fluid leaks, or chronic health issues—such as alcoholism, chronic heart, liver, or lung disease, or diabetes—are more vulnerable to severe complications from pneumococcal infections. Federal guidance stresses that these higher-risk groups should be vaccinated to prevent severe illness, permanent disability, or fatal outcomes from a pathogen that antibiotics do not always successfully treat.
