The bulletin also shows a critical picture of the incidence of AIDS in pregnant women: almost seven in every 10 pregnant women (67.7%) diagnosed with HIV are black. Since 2000, notification of pregnant women with HIV has been mandatory in the country in an attempt to stop the so-called “vertical transmission”, which occurs from mother to baby. The data shows, however, that this strategy needs to be strengthened. The incidence of AIDS among black people is even higher when considering younger people. Among people aged 14 with the disease detected, 71.2% are black. This proportion drops in the older age groups. AIDS is considered a “socially determined” disease, that is, associated with exclusion, which indicates the vulnerability of the black population in several social aspects. According to the Minister of Health, Nísia Trindade, it is necessary to take these aspects into account to promote access to health among this population. “All diseases are influenced by what we call social determinants and also environmental determinants. Not only infectious diseases, but also diabetes, cardiovascular diseases. So, it is very important to pay attention to the fact that the quality of health and health promotion go through a broad view of these social aspects” analyzes Nísia. During the bulletin launch event, authorities from the Ministry of Health classified the death rates from hypertension among black pregnant women as “unacceptable”. The index went from 36.4% in 2010 to 39.7% in 2020. “Everything the bulletin contains is explained by the persistence of racism. More than the cause, which I believe always needs to be deepened, we need to work in policies that lead us to actually overcome racism in our society, equity as a guiding principle”, argues the minister. In April, the Ministry of Health removed the option from the SUS (Unified Health System) notification fields that made it possible to classify the color of the person served by the SUS as “no information”. As a result, health professionals are required to indicate race/color, which, according to the folder, should improve the quality of the data. Compulsory notification of sickle cell anemia Minister Nísia Trindade also announced that the department will publish a technical note making notification of sickle cell anemia mandatory. The disease, which is genetic, causes red blood cells to lose their disc shape, becoming deformed. This leads to circulation problems and causes patients to have anemia, pain, injuries and become more susceptible to infections. The disease is more common among the black population. “This notification already exists in several states of the federation”, commented Nísia Trindade, remembering that there is a higher incidence of the disease in states such as Bahia and Tocantis. “Our orientation is for this to occur throughout Brazil. We already have technical tools for this, we need to record this data so that we can act better in caring for these people and (establish) other policies that are necessary”, explained Nísia Trinity.
