The global community is currently witnessing a dangerous divergence in public health: while medical breakthroughs and expanded access have saved millions of lives, those global health gains face threat of reversal due to systemic inequality, financing crises, and a critical lack of reliable data.
According to the World Health Statistics 2026 report released by the World Health Organization (WHO), the world is officially off track to meet any of the health-related Sustainable Development Goals (SDGs) by 2030. The findings paint a picture of “fragile progress,” where significant victories in infectious disease control are being undermined by rising environmental risks and the lingering structural damage caused by the COVID-19 pandemic.
As a physician, I have seen how clinical success in a vacuum—such as a new vaccine or a more effective antiretroviral—means little if the patient cannot afford the transport to a clinic or if the clinic lacks a reliable power source. The WHO’s latest data confirms this reality on a global scale: the tools for health exist, but the systems to deliver them equitably are fracturing.
The Paradox of Progress: Wins and Setbacks
The report highlights several areas of meaningful achievement. Between 2010 and 2024, new HIV infections dropped by 40%, with the WHO African Region seeing an even more dramatic reduction of 70%. Similarly, the number of people requiring interventions for neglected tropical diseases fell by 36% over the same period. There has also been a global decline in the consumption of both tobacco and alcohol since 2010.
Infrastructure improvements have also provided a critical foundation for health. Between 2015 and 2024, billions of people gained access to the basic environmental determinants of health:

However, these gains are countered by alarming reversals. Malaria incidence has increased by 8.5% since 2015, pushing the world further from its eradication targets. Preventable health risks remain stagnant. anaemia continues to affect 30.7% of women of reproductive age, a figure that has shown no improvement in a decade. Among children under five, the prevalence of overweight reached 5.5% in 2024, signaling a shift toward nutritional crises in developing regions.
The High Cost of Care and Systemic Failure
One of the most sobering aspects of the report is the stagnation of universal health coverage (UHC). The global UHC service coverage index rose only marginally, from 68 to 71, between 2015 and 2023. This lack of progress has devastating financial implications for the world’s most vulnerable populations.
In 2022, an estimated 1.6 billion people were either living in poverty or pushed into it specifically due to out-of-pocket health spending. This creates a vicious cycle where the cost of treating a preventable illness ensures the patient remains in the poverty that caused the illness in the first place. One quarter of the global population now faces significant financial hardship due to health costs.
This systemic fragility was laid bare by the COVID-19 pandemic. Between 2020 and 2023, the pandemic was linked to an estimated 22.1 million excess deaths—a figure more than three times higher than officially reported COVID-19 deaths. This massive loss of life not only reversed a decade of gains in life expectancy but also created “immunity gaps” in childhood vaccination coverage that are now triggering new outbreaks of preventable diseases.
The Data Blind Spot
A recurring theme in the report is the danger of “flying blind.” Effective public health requires precise data to allocate resources, yet the WHO reports a staggering gap in mortality reporting. As of the end of 2025, only 18% of countries reported mortality data to the WHO within one year, and nearly one third have never reported cause-of-death data.
Of the 61 million deaths estimated globally in 2023, only about one third were reported with cause-of-death information, and a mere one fifth had meaningful International Classification of Diseases (ICD) coded data. Without this information, health officials cannot accurately track the impact of air pollution—which contributed to 6.6 million deaths in 2021—or the effectiveness of new health interventions.
“Data gaps severely limit the ability to monitor real-time health trends, compare outcomes across countries, and design effective public health responses,” said Dr. Alain Labrique, Director for the Department of Data, Digital Health, Analytics and Artificial Intelligence.
The Path Forward
The WHO leadership emphasizes that these trends are not inevitable. Dr. Tedros Adhanom Ghebreyesus, WHO Director-General, noted that the data tells a story of “persistent inequality,” emphasizing that women, children, and underserved communities are still denied the basic conditions for a healthy life. The solution, according to the report, lies in securing sustainable financing and strengthening primary health care to build resilience against future emergencies.
The urgency is underscored by the fact that while maternal mortality has fallen by 40% since 2000, it remains nearly three times higher than the 2030 target. Under-five mortality has also declined by 51%, yet many nations remain dangerously off track.
