Innovative Home Hospital Program Expands Rehabilitation Access in Loir-et-Cher

Offering thorough, at-home care, the LNA Santé hospital-at-home (HAD) in Loir-et-Cher is pioneering a new sector focused on rehabilitation, addressing critical shortages in specialized care and expanding access for patients across the region.This initiative, launched in November 2023, is already demonstrating significant impact, wiht a dedicated team supporting hundreds of patients annually.

The growing demand for flexible healthcare solutions has driven this expansion. “From 2022, the idea of creating a rehabilitation sector has emerged within HADs throughout France,” explained a doctor specializing in physical medicine and rehabilitation at HAD Loir-et-Cher. “The objective is to compensate for the lack of rehabilitators and specialized structures nationally and to offer patients a rehabilitation course while remaining at home.”

Multidisciplinary Approach to Patient Recovery

The core of the program lies in its multidisciplinary care model. Unlike traditional rehabilitation settings, patients receive five rehabilitation sessions per week in the comfort of their own homes, delivered by a team of at least three therapists from diverse professions. This holistic approach addresses a wide spectrum of conditions, including:

  • Orthopedic issues: post-operative recovery, polytrauma, fracture rehabilitation.
  • Neurological conditions: stroke recovery,Parkinson’s disease,multiple sclerosis.
  • Oncology support: rehabilitation following cancer treatment.

This collaborative surroundings brings together specialists such as physiotherapists, neuropsychologists, dieticians, occupational therapists, teachers of adapted physical activity, psychomotor therapists, and speech therapists, working in conjunction with existing HAD professionals like psychologists, social workers, and nurses.

A Personalized Pathway to Wellness

To ensure optimal outcomes, the HAD Loir-et-cher employs a rigorous, patient-centered process. “Each treatment begins with an assessment at the patient’s home in order to build a personalized care plan, adapted to their pathology and their needs,” stated a senior official. “We then define the care to be implemented and mobilize the necessary professionals. A multidisciplinary meeting is held regularly for each patient in order to monitor their progress and adjust treatment if necessary.”

The program’s success is evident in its early results. Over the past year, the dedicated rehabilitation team has supported 260 patients, demonstrating the growing need for this type of service.

Expanding Awareness and Recruiting Talent

HAD Loir-et-Cher actively collaborates with local hospital services and private physicians to ensure seamless patient care. To further expand its reach, the association is hosting a series of “Afterwork Kiné” events on February 3 in romorantin, February 4 in Blois, and February 5 in Vendôme, specifically targeting self-employed physiotherapists.

“it is essential that they are aware of this system, especially since HAD patients can keep their usual healthcare professionals,” noted a nurse coordinator. These events also serve as a recruitment opportunity, as HAD Loir-et-Cher is currently seeking to bolster its team with additional physiotherapists, speech therapists, and teachers of adapted physical activity.
