IMSS doctors avoided a tragedy

The doctors on duty stabilized him and, in a surgery that lasted more than eight hours, they removed a section of vein from his left leg and connected it from the subclavian artery (which leaves the collarbone) to the humeral artery (from the shoulder) to give irrigation. Thanks to this action, the 21-year-old now retains his limb and still has vitality. The name of the procedure is anastomosis with saphenous vein.

According to testimony, everything was generated on Friday, January 13 during the evening shift. At that time young Alan was working on repairing a fertilizer conveyor belt. The glove on his right hand got stuck in one of the staples. At that moment the gang dragged him towards the roller and tore his arm.

The beneficiary assured that he never lost consciousness. As a safety mechanism, the machine stopped by itself and called by radio for help. With the support of his colleagues he walked down from the third floor and in a private ambulance he was transferred to HGZ No. 16.

Around 10 p.m. he arrived at the emergency department, was evaluated by trauma and was referred to the High Specialty Medical Unit (UMAE) No. 71, where a CT angiography was performed. After obtaining the result, they returned him to HGZ No. 16 with the amputation order. Instead, doctors Edwin Leopoldo Maldonado García and Javier Servín Castro took the risk of putting him in the operating room to perform the surgery. salvage surgery.

The anesthesiology specialist, Dr. Javier Servín, explained that Alan entered the operating room as a critical patient due to the amount of blood he had lost in the accident, plus the amount he would lose in the surgery and with the risk of presenting thrombosis as a compensation mechanism of the body.

Characteristics of the surgery performed by IMSS doctors

The intervention lasted eight hours, which is why it was necessary to apply general anesthesia with invasive management, as well as adequate pharmacology to maintain its signs and the saturation that the extremity required. In the end he was extubated and went to the postoperative care unit in stable condition, without requiring drugs to maintain blood pressure and in stable condition.

He commented that before starting any surgery An oxygen meter is always placed. In this case, because it was a rescue surgery, they put two: one in each hand and in this way they checked the level of perfusion of the damaged limb.

Anesthesiologist Javier Servín said he was surprised, fascinated and happy to see Alan’s success, since despite the low possibility of success he did not lose his limb.

Surgeon Edwin Leopoldo Maldonado explained that the saphenous vein It is located in the leg and is usually used to perform cardiac bypasses in open heart surgeries; However, medical literature indicates that it can also be used to replace a blood vessel, as in this case.

He added that the risk of the graft not working was high because the section was approximately 10 centimeters and is considered quite long, coupled with the fact that there was a risk of thrombosis; However, things turned out well.
