Lifestyle Intervention Slows Cognitive Decline, Improves Heart & Sleep Health, New Studies Show

A comprehensive lifestyle program designed to boost cognitive function in at-risk adults also delivers significant benefits for cardiovascular health, sleep quality, and may even offer protection against the biological hallmarks of Alzheimer’s disease, according to findings presented this week at the Clinical Trials in Alzheimer’s Disease (CTAD) annual meeting in San Diego.

New research from three National Institute on Aging-funded ancillary studies reveals the U.S. POINTER intervention, with its emphasis on structured support, offers “substantial and significant health benefits beyond improving cognition,” according to a statement from Maria C. Carrillo, PhD, Alzheimer’s Association chief science officer and medical affairs lead. “This positive relationship may multiply the beneficial impact of closely adhering to the structured US POINTER ‘recipe.’”

The U.S. POINTER Trial: A Holistic Approach

The U.S. POINTER trial, initial results of which were published in JAMA last July, tested a multifaceted lifestyle intervention. This included regular physical exercise, adherence to the MIND diet, engaging in computerized cognitive training alongside social and intellectual activities, and consistent health metric reviews coupled with personalized goal-setting sessions. The trial compared two approaches: a structured intervention with intensive support, and a self-guided method. Participants in the structured group demonstrated notably greater improvements in overall cognitive function compared to those in the self-guided group, equating to an estimated one to two years of slowed cognitive aging.

Improved Sleep Quality Through Targeted Intervention

The POINTER-zzz ancillary study focused on sleep outcomes, monitoring 780 U.S. POINTER participants with at-home sleep monitoring devices. The study revealed that nearly 65% of participants exhibited at least mild sleep apnea at the study’s outset. Those engaged in the structured intervention experienced a reduction of one to two respiratory disturbance events per hour of sleep compared to the self-guided group.

Recognizing that sleep disorders are frequently undiagnosed or untreated in older adults, and that poor sleep quality is linked to cognitive decline, researchers suggest sleep may be a crucial, modifiable risk factor for Alzheimer’s disease and related dementias. “We are excited about this finding, as it shows that the structured intervention improves not only cognition but also other behaviors that affect brain health, which may increase protection against dementia,” said Laura D. Baker, PhD, professor of internal medicine and public health sciences at Wake Forest University School of Medicine and Principal Investigator of the sleep ancillary study.

Enhancing Cardiovascular Health & Blood Pressure Regulation

The POINTER-NV study, involving 491 participants, assessed neurovascular function through comprehensive vascular health testing at baseline, 12 months, and 24 months. Utilizing ultrasound and continuous blood pressure monitoring, researchers evaluated blood vessel structure and function throughout the body and brain.

The structured intervention yielded significant improvements in the cardiovascular response to sudden blood pressure changes, indicating enhanced blood pressure regulation compared to the self-guided group. Furthermore, the intervention positively impacted multiple measures of blood vessel health in the aorta and carotid arteries. According to Principal Investigators Drs. Brinkley and Shaltout, the structured intervention “can improve the body’s ability to regulate blood pressure, which is crucial for proper brain blood flow.”

Biomarker Interactions: Protecting Against Alzheimer’s Pathology

POINTER-Neuroimaging represents the first large-scale investigation into the effects of a lifestyle intervention on biological markers of Alzheimer’s disease and dementia. Enrolling roughly 50% of the original trial participants, the study utilized MRI and positron emission tomography (PET) imaging to assess Aβ and tau protein accumulation.

The research demonstrated that participants with lower hippocampal volume or higher tau protein accumulation experienced greater cognitive benefits from the structured intervention compared to those with similar brain changes in the self-guided group. Notably, amyloid burden did not influence the degree of cognitive benefit. “Participating in the U.S. POINTER study’s structured intervention protected against the negative effects of tau tangle build up or smaller baseline hippocampal volume,” explained Dr. Susan Landau, Principal Investigator of the neuroimaging study. She added that individuals with amyloid buildup experienced the same benefits from the intervention as those without.

Implications for Alzheimer’s Prevention

These findings reinforce the potential of lifestyle interventions to address multiple risk factors for cognitive decline and dementia. As Carrillo stated, the U.S. POINTER intervention, with its structured support, provides “substantial and significant health benefits beyond improving cognition — and the benefits are in areas known to lower risk of cognitive decline and dementia.”

NIA Director Dr. Richard Hodes expressed encouragement regarding the early findings, noting they “offer valuable insights into the physiological mechanisms that may have contributed to the positive results of the U.S. POINTER trial.” The ancillary studies were funded through NIH grants: POINTER-zzz (R01AG064440), POINTER-NV (R01AG066910), POINTER-Neuroimaging (R01AG062689), and POINTER-Microbiome (U19AG063744). Results from a fourth ancillary study examining microbiome changes are expected in 2025.
