Measles Outbreak in Mexico Prompts Mask Recommendations, Sparks Debate Over Effectiveness
Mexico is grappling with a growing measles outbreak, with 2,642 confirmed cases reported as of 2026, prompting some state and local governments to reinstate mask mandates in schools and public spaces. However, the efficacy of this measure is being questioned by medical specialists who emphasize vaccination as the primary defense against the highly contagious disease.
Rising Cases and Regional Responses
The measles outbreak, which began in 2025, has led several states to take preventative action. Authorities in the State of Mexico, Oaxaca, Tlaxcala, Jalisco, and Hidalgo currently recommend the use of face masks to curb infections.
In the State of Mexico, mask-wearing is advised for all school personnel, both indoors and in areas where maintaining a safe distance is challenging. Tlaxcala and Oaxaca have similarly included masks and frequent handwashing among their preventative protocols.
Jalisco is implementing a more targeted approach. Héctor Raúl Pérez Gómez, the Secretary of Health of Jalisco, announced that face masks are required in schools within the Guadalajara Metropolitan Area and municipalities with the highest number of active cases, including Guadalajara, Zapopan, Tlaquepaque, Tonalá, El Salto, Tlajomulco de Zuñiga, and Ixtlahuacán de los Membrillos. Pérez Gómez also recommended mask use in workplaces throughout the state.
Further north, in Hidalgo, the municipal governments of Atotonilco de Tula and Progreso de Obregón have mandated masks in markets due to cases in neighboring municipalities. As of February 11th, merchants in Atotonilco de Tula are required to wear masks while operating flea markets, plazas, and serving the public. The local government has appealed for public cooperation, emphasizing the collective responsibility in protecting family health. Progreso de Obregón has implemented similar measures, mandating mask use on Sundays and Mondays at local tianguis (open-air markets).
The Debate Over Mask Effectiveness
While local authorities are implementing mask recommendations and mandates, some medical experts are skeptical of their impact on measles transmission. Dr. Alejandro Macías, in a recent interview with El Financiero, argued that masks offer limited protection against measles.
“If a child enters with measles to a room full of children who are not vaccinated, two or three are going to get infected, but if you ensure that they are vaccinated, the minors do not need anything, they are not going to get infected with measles,” Macías stated. He clarified that while masks are effective in preventing the spread of COVID-19, measles spreads through air, contact, and multiple other mechanisms, making mask-wearing a less reliable preventative measure.
IMSS Guidance on Mask Use
The Mexican Social Security Institute (IMSS) offers specific guidance on when mask use is recommended at the national level. According to the IMSS, masks are advised for:
- Individuals exhibiting symptoms of, or with a confirmed diagnosis of, measles, particularly when near others or leaving isolation.
- Caregivers, especially those without a complete vaccination schedule, when in direct contact with a sick person or their environment.
- Medical and nursing staff during the care of suspected or confirmed cases.
- The general population residing in areas with a high incidence of infection, as an additional preventative step.
The IMSS recommends using three-layer or N95 masks, ensuring a snug fit over the nose and mouth, and proper hand hygiene after removal.
Regional Hotspots and Vaccination Urgency
The majority of Mexico’s measles cases are concentrated in just seven states. Jalisco accounts for the largest share with 1,245 cases, followed by Chiapas (238), Mexico City (120), Sinaloa (118), Colima (50), Tabasco (35), and Nayarit (23). These figures represent 85% of the total confirmed cases nationwide.
The ongoing outbreak underscores the critical importance of vaccination as the most effective strategy to combat measles. Public health officials continue to urge residents to ensure they and their families are up-to-date on their measles vaccinations to protect themselves and vulnerable populations.
