Over 40% of ovarian cancer cases in England are diagnosed after emergency hospital admissions, according to a national study analyzing 28,204 cases between 2017 and 2021. The research highlights stark disparities linked to age, frailty, and neighborhood deprivation, with younger and older women, as well as those in deprived areas, facing higher risks of late-stage diagnosis.

The findings, published in BMJ Oncology, reveal that 40.3% of women diagnosed with ovarian cancer in England between 2017 and 2021 were identified through emergency admissions, a rate that underscores significant challenges in early detection. Women aged 18–29 had a 42.7% emergency diagnosis rate, while those over 80 faced a 54.9% rate, compared to 36.2% for women aged 60–69. Severe frailty was another critical factor, with 68.6% of women with the highest frailty scores diagnosed after emergencies versus 29.2% for those with the lowest scores. Neighborhood deprivation also played a role, with 44.2% of women in the most deprived areas diagnosed via emergency admission versus 37.8% in the least deprived.

These disparities align with broader trends in cancer care. The study found that only 13.7% of women diagnosed through emergency admissions had early-stage cancer, compared to 39.2% of those diagnosed through non-emergency routes. Younger and older women, frailer women, and those living in more deprived neighborhoods were at an even higher risk of being diagnosed only after the symptoms of their cancer were so severe that an emergency admission was deemed necessary, the authors wrote.

International Context and Calls for Action

The issue is not confined to England. The study authors noted that emergency diagnosis rates for ovarian cancer range from 20% to 50% in countries like the U.S., Australia, Denmark, Norway, Canada, and New Zealand. Concerted action, where possible, with support from international collaborations, is needed to improve referral and diagnostic pathways, the researchers stated. This includes increasing patient awareness, improving symptom recognition, and streamlining diagnostic processes.

Richard Simcock echoed these calls, pointing to the NHS England Cancer Plan’s goal of diagnosing 75% of cancers at stages 1 or 2 by 2035. However, the study found that only 29.6% of ovarian cancers were diagnosed at early stages, with 13.7% of emergency-diagnosed cases falling into this category. There is much work to do here, Simcock said, stressing the need for targeted interventions to address systemic gaps in care.

The risk of being diagnosed with ovarian cancer after an emergency admission was higher in younger women, despite having higher rates of early-stage low-grade cancers—factors associated with lower rates of ovarian cancer diagnosis following emergency admission, the study authors noted. This paradox highlights the complexity of ovarian cancer detection, where symptoms often mimic benign conditions and no reliable screening exists.

NHS Plans and the Path Forward

Scotland’s 10-year Cancer Strategy 2023–2033 aims to reduce late-stage diagnoses (stages 3 and 4) from 42% to 24%, a target that underscores the urgency of addressing emergency admissions. The study’s authors recommended developing interventions to identify high-risk patients in both secondary care and community settings. Interventions need to be developed and implemented to identify the women who are most at risk, they wrote.
