Tramadol works by affecting multiple pathways in the brain, easing pain through a dual-action mechanism. Doctors frequently prescribe it for both short-term and chronic moderate to severe pain. Its perceived lower risk of addiction and fewer side effects compared to traditional opioids contributed to its increasing popularity among both physicians and patients.

What Previous Studies Overlooked

While tramadol has been studied before, earlier reviews often focused solely on pain reduction, neglecting a thorough assessment of safety. Others examined limited types of pain, leaving critical gaps in our understanding. To address these shortcomings, researchers analyzed randomized clinical trials published up to February 2025, comparing tramadol to a placebo in individuals experiencing chronic pain, including cancer-related discomfort.

A Detailed Look at the Trials

The analysis encompassed 19 clinical trials involving 6,506 participants. These trials focused on a range of conditions: five examined nerve pain, nine osteoarthritis, four chronic low back pain, and one fibromyalgia. The average participant was 58 years old, with ages ranging from 47 to 69. Treatment durations varied from 2 to 16 weeks, with follow-up periods lasting 3 to 15 weeks—a crucial factor, as many side effects and long-term risks take time to manifest.

Modest Relief, Significant Risks

The results painted a clear picture: tramadol did offer some pain relief, but the effect was small—too small to meet conventional standards for clinically meaningful improvement. While some patients may have experienced a slight benefit, it likely didn’t translate into substantial improvements in daily functioning.

Eight trials tracked serious side effects over 7 to 16 weeks, revealing tramadol was linked to approximately double the risk of harm compared to a placebo.

This heightened risk was primarily driven by heart-related issues, including chest pain, coronary artery disease, and congestive heart failure. A potentially increased risk of certain cancers was also observed, though researchers cautioned that the short follow-up periods limited the certainty of this finding.

Common Side Effects

Across all trials, tramadol was associated with more frequent occurrences of nausea, dizziness, constipation, and sleepiness. While these side effects may seem minor, they can significantly impact quality of life, particularly for older adults.

The study authors also noted limitations within the trials themselves, with many outcomes carrying a high risk of bias. This suggests the benefits of tramadol may have been overestimated, while its harms underestimated.

Opioids in Context

These findings arrive amid a broader opioid crisis. Approximately 60 million people worldwide experience the addictive effects of opioids, according to the study’s authors. In 2019, drug use contributed to roughly 600,000 deaths globally, with opioids implicated in nearly 80 percent of those fatalities and overdoses accounting for about 25 percent. In the United States, opioid-related overdose deaths climbed from 49,860 in 2019 to 81,806 in 2022.

“Given these trends and the present findings, the use of tramadol and other opioids should be minimized to the greatest extent possible,” the researchers wrote.

A Pain Relief Trade-Off?

After careful consideration of the evidence, the research team concluded that tramadol may provide a slight reduction in chronic pain, but likely at the cost of increased risk for both serious and non-serious adverse events. “The potential harms associated with tramadol use for pain management likely outweigh its limited benefits,” they noted.

For a drug so widely prescribed and trusted, this conclusion demands a serious reevaluation of tramadol’s role in chronic pain care—not because it’s ineffective, but because its benefits may simply not justify the risks.
