Adults with type 2 diabetes who closely follow a Mediterranean lifestyle—including plant-based eating, regular physical activity, and adequate rest—experience a 27% lower risk of major adverse cardiovascular events, according to a UK Biobank study published in eClinicalMedicine.
Cardiovascular disease remains the primary complication for patients diagnosed with type 2 diabetes, who face up to a 2.5-fold higher risk of heart failure, ischemic stroke, myocardial infarction, revascularization, and cardiovascular death, as noted in recent health data. While individual healthy habits like diet and exercise are widely recommended, a large-scale analysis has examined how a comprehensive lifestyle approach alters those long-term risks.
Tracking 3,600 British Adults Across a Decade
Researchers examined data from 3,612 British adults aged 40 to 70 with type 2 diabetes who participated in the UK Biobank project. Participants completed baseline assessments between 2009 and 2012, answering touchscreen questionnaires on physical activity, sedentary behavior, social habits, and rest, alongside repeated 24-hour dietary assessments collected via the Oxford WebQ questionnaire.
Adherence was measured using a modified Mediterranean Lifestyle Index, known as MEDLIFE, which scores participants across 25 items divided into three distinct blocks: Mediterranean food intake, Mediterranean dietary habits, and a combined category covering conviviality, rest, physical activity, and social habits. Because certain components like olive oil consumption and eating with company were unavailable in the UK Biobank data, researchers utilized a modified version of the index.
The study followed participants for a median of 10.5 to 12.1 years. During that period, researchers recorded 737 major adverse cardiac events, 645 deaths from any cause, and 171 deaths specifically attributed to cardiovascular disease.
Measurable Reductions in Cardiovascular Risk and Mortality
The findings demonstrate a clear gradient of protection for individuals who maintained robust adherence to the lifestyle pattern. Participants in the highest adherence group had a 27% lower risk of major adverse cardiac events—which encompass myocardial infarction, ischemic stroke, heart failure, revascularization, and cardiovascular death—compared to those in the lowest adherence group.

Survival outcomes showed even sharper divergence. Each two-point increase in the MEDLIFE score was associated with a 12% reduction in major adverse cardiac event risk, a 9% reduction in all-cause mortality, and a 21% reduction in cardiovascular mortality.
Individual blocks of the lifestyle index contributed distinct protective effects. A two-point increase in the food-consumption component was linked to a 10% lower risk of cardiac events, while the physical activity, rest, and social habits component associated with a 17% lower risk of cardiac events, a 14% lower risk of all-cause mortality, and a 25% lower risk of cardiovascular mortality.
The UK Biobank findings align with extensive European clinical research exploring how structured lifestyle interventions modify metabolic disease. The PREDIMED-Plus trial, the largest nutrition trial conducted in Europe involving University of Navarra researchers and over 200 investigators across Spanish universities and hospitals, evaluated 4,746 adults between ages 55 and 75 who had overweight or obesity and metabolic syndrome.

That six-year randomized study, published in the Annals of Internal Medicine, found that participants who combined a Mediterranean diet with a daily calorie reduction of about 600 calories, moderate physical activity, and professional weight-loss support reduced their risk of developing type 2 diabetes by 31% compared to a control group following a traditional Mediterranean diet without calorie restrictions.

While observational studies like the UK Biobank analysis cannot establish definitive causality due to self-reported behavioral data and potential residual confounding, the convergence of large-scale cohort data and randomized clinical trials underscores that integrating social connection, rest, and physical activity with traditional plant-forward nutrition creates a potent framework for managing chronic disease risk.
