The UMCG will lead an extensive Dutch/Belgian study into a new way of treating patients with an acute cerebral infarction. In these patients, in whom a clot is removed from the cerebral artery, a stent is simultaneously placed to remove a narrowing in the carotid artery.

Intervention neurologist Maarten Uyttenboogaart of the UMCG is leading this research, together with neurologists Paul Nederkoorn from the Amsterdam UMC and Robin Lemmens from the UZ Leuven (Belgium). They will receive a subsidy totaling €2.3 million for this; €1.4 million from ZonMw and €0.9 million from the Belgian KCE. 17 hospitals in the Netherlands and 9 hospitals in Belgium are participating in the study and it will last four years.

Patients with an acute cerebral infarction due to a blocked cerebral artery are currently treated with a so-called intra-arterial thrombectomy (IAT). A clot is removed from the cerebral arteries using catheter techniques. When the blood supply is restored in time, this treatment reduces the risk of permanent disability after an acute cerebral infarction.

Narrowing in carotid artery
One fifth of the patients undergoing this IAT treatment have a narrowing of the carotid artery that supplies blood to the brain. This narrowing is also the cause of the cerebral infarction in them. These patients often have to be operated on again within 1 to 2 weeks after the procedure because of the cerebral infarction in order to remove this narrowing of the carotid artery. This is to prevent a new stroke.

Remove clot and place stent at the same time
In this new CASES study, the researchers are investigating whether this narrowed carotid artery can also be treated with a stent during the IAT procedure. This is a tube that can be placed in a patient’s blood vessel to keep it open. Doing this at the same time may offer some major benefits to the patient. A patient then no longer needs to be operated on again with any risks that such an intervention entails. Furthermore, the cause of the cerebral infarction is treated immediately and the cerebral blood flow immediately improves. A possible disadvantage for a patient is that after the cerebral infarction he has to use two blood thinners daily for a longer period of time; now it is two weeks, with the simultaneous operation it is three months. A completely different benefit may be that simultaneous treatment ultimately leads to lower healthcare costs.

Hopefully better outcomes for patient
Uyttenboogaart expects a lot from this research: ‘Every year, more than 2000 people in the Netherlands who are affected by an acute cerebral infarction are treated with an IAT. I hope we can demonstrate that immediate stent placement leads to better patient outcomes. Then this will become the standard treatment for patients who have had an acute cerebral infarction due to a narrowed cerebral artery.’

Extensive research
This is a study that will last 4 years. The study is being conducted in 17 hospitals in the Netherlands and 9 hospitals in Belgium. The UMCG is the coordinating center of this study, Maarten Uyttenboogaart is the principal investigator. It is expected that approximately 600 patients with acute cerebral infarction will be included in the study. The research was made possible by a cerebral infarction from ZonMw/KCE’s BeNeFit program worth a total of €2.3 million. The research is part of the renowned CONTRAST research consortium in which all scientific research into acute cerebral infarctions and cerebral haemorrhages is bundled in the Netherlands.
