Peritoneal carcinomatosis is a form of digestive or gynecological tumors spreading into the peritoneum. Experts point out that more specialized units are needed for better treatment of patients and point out that this type of cancer is being talked about more.

In Spain there are around 40 units specialized in the treatment of peritoneal carcinomatosis, according to what they say at EFEsalud the heads of the Peritoneal Oncology Unit of the MD Anderson Cancer Center Hspettin Madrid, doctors Santiago González and Gloria Ortega.
Ortega and González attended the Conference yesterday “Peritoneal oncology. We are celebrating 20 years”which was attended, for the occasion, by several experts and patients of the 20th anniversary of this specialized unit from the hospital.
Peritoneal carcinomatosis
Peritoneal carcinomatosis is the metastasis to the peritoneum of tumors of the abdominal cavity: digestives such as those of the colon, stomach, small intestine and appendix; and of the gynecological onemainly of the ovaries and fallopian tubes, as explained by Dr. Ortega, oncological surgeon.

These are secondary tumors, but the tumor can also initially originate in the peritoneum, the tissue that lines the abdominal cavity, although it is less frequent, according to Santiago González.medical director and head of the surgical oncology service at MD Anderson Hhosten Madrid, hospital with which EFE collaborated in organizing the day, together with the Peritoneum Association.
The prevalence and incidence vary depending on the primary stroke that causes it.
The symptoms are not specific -abdominal discomfort, bloating and difficulty digestion, among others- this is why when the metastasis is detected it is already in an advanced stage.
The diagnosis of peritoneal carcinomatosis is usually made by imaging, such as ultrasound, CT scan or MRI. But if this is not possible and there is a high suspicion that the tumor has spread, laparoscopy is used, which consists in introducing, after an incision, a camera near the navel.
Treatments
More than 30 years ago “there was no cure” for this type of tumor. However, over the last three decades, treatment techniques have been developed in “selected patients,” who will initially respond better cytoreductive surgery combined with intraperitoneal chemotherapy -inside the abdomen-.
“We surgically remove all tumor implants that we can see,” explains González, adding that to achieve this it may be necessary to remove parts of the peritoneum or some organs.

“Once there is nothing left, we apply the chemotherapy directly to the abdomen which we heat to 42 and a half degrees, more or less. We usually keep it for about 90 minutes,” he continues.
This combination of cytoreductive surgery and intraperitoneal chemotherapy with hyperthermia “is what has been shown in select patients” who “can actually survive much longer.”
However, not all patients are candidates for this surgery.and these are currently treated with intravenous palliative chemotherapy “which today has better results than 30 years ago”. There is also the possibility of chemotherapy with pressurized intraperitoneal aerosol, among others.
The specialized units
This is care that patients benefit from in specialized units, such as the one for which doctors Ortega and González are responsible and which is made up of different professionals from medicine, surgical oncology, radiology, nursing, anesthesiology and pathology, among other specialties.
“There are still patients suffering from peritoneal carcinomatosis in Spain and in many parts of the world who do not have access to a specific unit of this type, it happens less and less, but it still happens”, underlines González, co-head of the Peritoneal Oncology at MD Anderson Hosten Madrid.
The expert specifies that if in cases of breast or colon cancer patients are referred to specific structures, “there must be awareness that peritoneal oncology units exist” and that “it is necessary to create others” so that each patient can at least be valued and offered what they might need.
Dr. Ortega is of the same opinion, who underlines that today it is known that a single specialist cannot cover everything related to the care of a patient “and even less in a complicated situation like this”.
A specialized unit “It’s essential”Ortega agrees and argues that they are necessary so that patients do not miss “their best opportunity for treatment.”
The voice of patients
In fact, one of the main requests from patients is to be able to be referred to specialized departments, as indicated to EFEsalud. the president of the Peritoneum Association, Sandra Lara de Lizwho overcame a tumor of this type a few years ago.

“Many times when they (patients) contact us, they say: how come no one told me about this? How is it that at no point were we informed of the existence of these specialized peritoneal oncology units? Because many times, even in the healthcare sector itself, you don’t know that they exist,” he claims.
For the president of the Peritoneum, it can make “a big difference” to the patient whether or not he is treated in a specialized unit. In the association’s website You can find hospitals that have them.
Likewise, like the experts, he emphasizes the importance of talking about these tumors because “Lives could be saved”.
