Exposure to light levels above 3 lux during nighttime sleep is linked to measurable, subtle changes in the heart’s structure and function, according to a study published in the European Heart Journal. Researchers analyzed wrist-sensor data and cardiac MRIs from more than 11,000 participants to examine how ambient nighttime illumination affects cardiovascular health.

Cardiac Remodeling Linked to Nighttime Illumination

A growing body of research has previously established a connection between sleeping in illuminated environments and an elevated risk of coronary artery disease, heart attack, stroke, and heart failure. However, a study led by researchers at Tulane University provides the first specific evidence regarding how nighttime light exposure affects the physical structure and functional capability of the heart.

The study utilized health and lifestyle records from the UK Biobank, examining 11,071 participants who wore light and motion sensors on their wrists for a seven-day period. Three years later, these individuals underwent cardiac MRI scans to provide detailed imaging of their heart chambers and muscle performance. Researchers compared participants exposed to light levels exceeding 3 lux against those sleeping in almost complete darkness. Three lux is a measurement of illumination that would still be a very dim room.

The findings revealed that even very dim nighttime illumination can influence cardiac health. Lu Qi, a professor at Tulane University’s Celia Scott Weatherhead School of Public Health & Tropical Medicine who led the research, noted that participants in the highest light-exposure category exhibited measurable physical changes in their heart tissue.

“This is the first study of its kind, and it shows that exposure to higher levels of light at nighttime is linked to cardiac remodelling. This is where the structure and function of the heart changes and we typically see it in response to chronic stress or injury to the heart. It’s our body’s way of adapting to that stress, but ultimately it weakens the heart and can lead to heart failure.”

Specific Structural Changes in the Heart Chambers

The physical alterations observed in participants exposed to higher nighttime light levels are classified medically as remodeling. Specifically, individuals in the highest exposure group demonstrated a 2.4% larger left ventricle, a 1.5% thicker heart wall, and a 1.9% reduction in the heart’s overall ability to contract compared to those sleeping in dark rooms. Additional structural shifts were identified in the right ventricles and left atria.

While individual metric shifts appeared modest, researchers emphasized their cumulative impact. Participants with the highest light exposure showed a 29% increased risk of heart failure, a 24% higher risk for heart attack, a 33% elevated risk for stroke, and a 26% increased risk of cardiovascular death compared to the group sleeping in the darkest rooms.

Common household sources contributing to this nighttime exposure include street light leaking through window shades, clock radio illumination, and small LED indicator lights on appliances such as air purifiers or humidifiers. Kristen Knutson, an associate professor of sleep medicine at Northwestern University’s Feinberg School of Medicine who was not involved in the study, pointed out that closed human eyelids remain capable of detecting these ambient light sources.

Circadian Rhythm Disruption and Sleep Deficits

First, nighttime light exposure frequently degrades sleep quality and shortens total sleep duration.

The study does carry certain methodological limitations. Wrist sensors recorded light exposure for only a single week.

Medical Community Response and Public Health Stakes

In an accompanying editorial published alongside the findings, Thomas Münzel, an environmental cardiologist at the University Medical Center of Johannes Gutenberg University in Mainz, Germany, along with his colleagues, argued that ambient bedroom darkness should be treated as a primary health metric.

The editorial team urged clinicians—particularly those treating patients with heart failure or atrial fibrillation—to actively inquire about patients’ sleeping environments, bedroom darkness levels, night-shift work habits, and evening screen usage after sunset. From a broader perspective, researchers noted that light pollution represents an environmental hazard that municipalities can address directly through shaded fixtures, warm-spectrum bulbs, and dimming or motion-activated street lighting.

Practical Adjustments and Future Research Directions

For individuals seeking to modify their home sleeping environments, experts recommend several straightforward and low-cost interventions. These steps include installing blackout curtains to block outdoor illumination, utilizing warm-colored bedside lighting, and covering standby LED indicator lights on electronics.
